SUB-ACUTE CARE

At HealthBridge Children’s Hospital our Sub Acute Care program offers specialized services to children, adolescents and young adults who are medically fragile and technologically dependent whose recoveries may be slow or indefinite. The goal is to integrate complex medical care while providing normalcy and the best quality of life possible.  Our size and unique care setting allows each child to receive focused attention for their special needs. We are committed to assisting children and their families through medically challenging times with the support of a dedicated staff in a warm nurturing environment. This program can be short or long term.

Children who can benefit from placement at HealthBridge include those with:

  • Ventilator Dependence

  • Premature Birth Complications

  • Birth Anomalies

  • Brain or Spinal Cord Injuries

  • Neuro Muscular Diseases

  • Respiratory Disorders

  • Complex Wounds

  • Muscle Weakness

  • Tracheostomy

  • Genetic Disorders

  • Dependence on Gastrostomy/Jeuostomy Tube 

 Approach to Treatment

Upon admission, every child who comes to HealthBridge is evaluated by a team of medical professionals who develop a comprehensive, individualized care plan. Depending on the severity of the illness or injury and response to treatment, patients can progress through different levels of care to achieve their optimal level of function. Because of our Acute Care capabilities, Sub Acute patients have access to more intense services if needed such as additional medical supervision, telemetry and rehab.   Extensive family training, home adaptation, and access to community resources are provided for those parents wishing to transition their child home. HealthBridge will continue to be the child’s home if circumstances are such that home placement is not possible and complex care and support is still required.
